Special Summon Monsters

A monster that can Special Summon a card, or Special Summon itself will only apply to the hand unless otherwise stated. HOWEVER for the sake of not having to errata a card for clarifications sake, please will you specify - a) where a card Special Summons itself from and b) where the Special Summon target comes from

Who what where when? Is this just a discussion or a decree?

More decree, since there have been far too many misinterpretations on this subject, leading to may people unsure on how their cards actually work.
